My customized includes and excludes don't seem to be working properly.
Answer:

If you are using TSM 4.1.2.15 for the Macintosh, and you have modified your include/exclude rules, there is a good chance that you might not be backing up what you intended. There was a bug in TSM 4.1.2.15 for the Macintosh.

According to Tivoli, the Fixtest code IC31746 fixed "Reversed Include/Exclude list after import of backup preferences." What this means is that if you upgraded from a previous version of TSM using the old preferences, or if you clean installed version 4.1.2.15 and modified your default Include/Exclude list, TSM processed them in reverse. Essentially, you would be backing up everything you didn't want to back up, and NOT backing up everythig you wanted backed up.

To fix this problem, upgrade to TSM 4.2.1.1.
